The Five Factor / Life Academy Corporation team uses the form to understand your interest in Big Five Certification and organize its response.
What data we record
Your name, email, location, education level, field of study, responses about your goals and circumstances, preferred contact method and any optional fields you complete. We also record the visit source, campaign parameters, submission date and your authorization. To reduce automated submissions, we may temporarily retain an identifier derived from your connection address.
How we use this information
To respond to your request, provide guidance about the program, notify you of cohorts according to your preferences and evaluate outreach results. Submitting the form does not enroll you. Access to responses in the management dashboard is restricted to authorized team members.
Your choices
You choose how you wish to continue and may request corrections, deletion of your data or an end to further contact through the Five Factor support channel. Please mention that your request concerns the certification interest form.
